When You Leave

Sometimes the prospect of coping at home after a stay in hospital can seem overwhelming. That’s why we have a dedicated Case Manager to help you plan for leaving the hospital. They can liaise with community organisations and organise post discharge needs and can be contacted on 8408 2284. Alternatively, a staff member can contact the Case Manager for you while you are still in hospital.

If you have any concerns regarding planning for your needs post-discharge, we recommend you contact the Pre-Admission Clinic on 8408 2014 before coming to St Andrew’s.

Please note that discharge time is 08.30 - 10.00am. However if you are unable to be collected by this time comfortable sitting rooms are available for your use.

Either one of our nurses or our Physiotherapist will meet with you to discuss your requirements and will organise the appropriate equipment needed. A range of items may be hired from external companies, including walking frames, seat raisers, sheepskins and bedpans.
